The temple worships the presiding deity, Attukal Amma, in three divine forms: Maha Saraswati, symbolizing knowledge; Maha Lakshmi, bestowing wealth; and Mahakali/Durga/Parvathy, representing power. According to legend, Attukal Amma is believed to be Kannaki, the courageous heroine of the epic 'Silapathikaaram' by Ilanko Adikal, adding to the temple's historical significance.

The temple's Gopuram depicts stories of Kannaki, while intricately carved figures of Mahishasuramarddini, Goddess Kali, Rajarajeswari, Sree Parvathy with Lord Paramasiva, and other depictions of the Goddess adorn the premises. The "Desavathara," showcasing the ten incarnations of Lord Vishnu, is a major attraction.
